Entrants must be eighteen (18) years of age or older and legal residents of the United States. Void where prohibited. The Sweepstakes is open only to MDR customers and previous customers with verified email addresses. Employees of MDR and its affiliates, as well as the immediate family (spouse, parents, siblings, and children) and household members of those employees, are not eligible to enter.

Entry Period

The Sweepstakes begins at approximately 8:00am est as indicated on the invitation distributed by MDR and within the the survey. (the "Entry Period"). Entries submitted before or after the Entry Period will not be eligible. MDR's computer is the official time keeping device for the Sweepstakes.

How to Enter

Complete the survey and verification using your current business email address and information that match the information on file for the verified MDR customer.  There is a limit of one (1) entry per person and you must use your verified e-mail addresses to enter. No other methods of entry will be accepted. The use of an agency or any automated system to enter is strictly prohibited, and MDR reserves the right to disqualify any entries received through such methods, as determined by MDR, in its sole discretion.


On or about 12:00pm EST on date indicated in the invitation and in the survey, MDR will email the winner(s) based on the quantity indicated in the survey and by their order of completion of the survey. The odds of being selected depend on the number of entries received. MDR or its agent will attempt to notify potential winner(s) via the verified email address provided within 5 business days of the drawing. If a winner cannot be contacted or is disqualified for any reason, MDR reserves the right to determine an alternate winner or not to award the winner’s prize, in its sole discretion.

Requirements of Potential Winner(s)

Except where prohibited, a potential winner may be required to complete and return an affidavit of eligibility and liability/publicity release. If a potential winner fails to sign and return these documents within the required time period, an alternate potential winner may be selected in his/her place in a random drawing of all remaining entries received. Only three (3) alternate potential winners may be contacted.


The winners will be awarded the incentive on or before the date indicated in the survey. Prizes will be emailed to the winners at the verified email address provided on the registration form. This drawing is not sponsored by or affiliated with Amazon. A winner is responsible for all expenses not specified above, including without limitation, paying all federal, state, local and applicable income taxes and any and all other costs and expenses not listed above. Any prize details not specified above will be determined by MDR, in its sole discretion. All prizes are awarded as-is without any warranty or guaranty, either express or implied by MDR. A prize may not be transferred and must be accepted as awarded. General Conditions

In the event that the operation, security, or administration of the Sweepstakes is impaired in any way, MDR may, in its sole discretion, either: (a) suspend the Sweepstakes to address the impairment and then resume the Sweepstakes; or (b) award the prize(s) at random from among the eligible entries received up to the time of the impairment. MDR reserves the right, in its sole discretion, to disqualify and seek damages from any individual who tampers with the operation of the Sweepstakes, violates these Official Rules, or acts in a disruptive or unsportsmanlike manner. MDR's failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.
